New Revision Details

Form Name:

The form name carries forward — edit only if the name genuinely changed

Number of Sections:

Leave empty to inherit the current revision's workflow, or configure a new one.

Assign Sections (optional):

Select a template first

Upload Revised Form Document: *

Upload the new version of the form (PDF or Word document)
